Student Athlete Information
Each student athlete is required to have a physical examination prior to any participation in any intercollegiate sport. This physical must be done by a KSU team physician at scheduled times dictated by the athletic training staff. This physical is given free of charge to all student athletes at the school. The final decision on physical qualifications or reason for rejection is the responsibility of the team physician or athletic trainer. The team physician and athletic trainer also make the decision on when an athlete may return to competition after an injury. INSURANCE
All athletes are required to have a current and valid form of Primary Medical Accident Insurance Coverage that covers sports injuries. Medicare and student health insurance from the university are not acceptable forms of primary insurance. A photo copy (front and back) of their health insurance card needs to be provided before a student athlete is allowed to participate. If your son/daughter does not have insurance, the KSU sports medicine department can provide you with options of policies to purchase. The athletic accident insurance that Kentucky State University provides for your son/daughter covers injuries while participating in the play or official team practice of intercollegiate sports. This is a secondary policy and only comes into effect after your primary insurance has acted. If a student-athlete’s primary insurance coverage is out of state or through an HMO or managed care, KSU requires that student-athlete and/or parent guardian to change the primary care physician to the KSU team physicians (Dr. Scott Mair or Dr. Robert Hosey). This will allow the student athlete to have a network of physicians to receive medical care and avoid likely delays or inability to receive medical care.
MEDICAL BILLS
Accidents do occur and we attempt to provide our athletes with the very best care. Medical bills may be incurred when the athlete is treated for injury due to athletic participation. All medical bills for your son/daughter incurred as the result of an accident in the intercollegiate sports program will be sent directly to your son/daughter or to your home address. All bills are to be filed within 52 weeks (one calendar year) of the date of injury. If bills are not submitted with in this time frame, KSU will not be responsible for any of the bills.
CLAIMS PROCEDURE
All medical bills will be submitted to your primary insurance first. They will do one of two things: 1. Honor the claim and pay all or portion of the bills incurred. 2. Not honor the claim and send you a letter of denial.
If there remains a balance after your primary insurance plan has contributed towards the claim, send the claim sheet or Explanation of Benefits (EOB) from the insurance company and a copy of the itemized bills incurred to KSU Athletic Training. If you receive a letter of denial from your primary insurance, then send the letter of denial and a copy of the bills incurred to KSU Athletic Training. It is in your best interest to have the claims settled promptly since all the bills incurred are in your name.
